The response cards and transcript/reference envelopes are not included here. You may call or write the Office of Graduate Admissions to have them sent to you, or you may use plain white envelopes, and we will add the response cards to your file upon receipt of the application. If you choose to use your own envelopes, be sure that they are sealed and signed across the back as indicated in the instructions.
