Introduction of New Forms

ROHP is making available more of its medical forms on its website. You will continue to see more medical forms added to the ROHP library over time.

How do I access these new forms?

These forms can be downloaded from the ROHP website by clicking on any of the Form titles and saved on your local hard drive or other storage device. When completed, the form(s) should be sent to ROHP either by email to rohp@bu.edu or faxed to the ROHP secure electronic fax number 617-977-8788.

Rabies Vaccine Series Consent Form

This consent form is to be used by laboratory and animal care workers whose occupational risk assessment includes working with or exposure to the rabies virus and/or animals known to carry the rabies virus, and the medical surveillance for this type of activity recommends the rabies vaccine. The rabies vaccine is made available to these personnel at no cost.

Preventive Vaccination (No Exposure):

The rabies vaccine as a preventive measure is given in three doses at the following times:

Dose 1:  When appropriate

Dose 2:  7 days after Dose 1

Dose 3:  21 days or 28 days after Dose 1

Vaccination After an Exposure:

Anyone who has been bitten by an animal, or who otherwise may have been exposed to rabies, should clean the wound and seek medical treatment immediately.

A person who is exposed and has never been vaccinated against rabies should get four doses of the rabies vaccine:

Dose 1:  Immediately

Dose 2:  3rd day after Dose 1

Dose 3:  7th day after Dose 1

Dose 4:  14th day after Dose 1

The Rabies Immune Globulin shot should also be administered at the same time as Dose 1 of the rabies vaccine.

A person who is exposed and was previously vaccinated should receive two doses of the rabies vaccine:

Dose 1:  Immediately

Dose 2:  3rd day after Dose 1

The Rabies Immune Globulin shot is not needed if someone was previously vaccinated.
