Adding Access to Animal Care Facility
Forms required
The following forms are submitted to Occupational Health dept. in order to obtain medical clearance.
Training and clearances required for general access
- LASC New Researcher Orientation Session with Facility Tours
This introductory session is required for access to the animal facilities. Click on the link for dates and times. Pre-registration is required as space is limited, cut off for registration is noon time the day before Orientation. Please contact LASC to register. - IACUC Orientation offered monthly
- W-9 Rodent Barrier Orientation
This orientation session is required for access to the W-9 rodent barrier facility. Please fill out this form. - Lab Safety training session
Call OEHS at 638-8830 or visit their web site to find out the date and time of their next session. - Occupational Health Clearance
You must complete an animal users health questionnaire and respirator clearance form and submit them to OEHS. In some cases, an exam, a TB test, or vaccinations may be required for clearance. - Your name must be listed on an approved IACUC protocol
The Principal Investigator can add new personnel via the IACUC's online protocol submission web site (INSPIR).
Access to special areas
- W-9 rodent barrier facility
Barrier Orientation session is now being done right after the New Researcher Orientation Session. - BL2 Biocontainment facility
ABSL-2 safety orientation session (scheduled as needed). - Non-human primates, NHP blood, Tissue, and Herpes B Training
(offered Monthly, check events calender). - Rodent Quarantine
Rodent Quarantine Orientation, access is by appointment only (schedule as needed). - 670 Albany St. Rodent Barrier Facility-Orientation and Training
Scheduled thru LASC Office
The LASC will process LASC facility access requests only after verifying that all of the above requirements have been met. Your ID card should work within 2 business days of our receipt of all required forms.
Revoking Access to the Animal Facility
It is the responsibility of the Principal Investigator to ensure that the LASC Business Office is notified whenever an employee's access to the LASC facilities should be revoked. Thank you for your cooperation with this very important security issue.
