- Addendum B: Detailed Information on a Project related to a Disclosed Significant Financial Interest (Word)This form is filed by investigators who have filed a positive disclosure on a project or protocol. The form provides additional data needed to inform the review by the relevant Advisory Committee on Investigators Conflicts of Interest. The investigator receives a request from the Committee to file the form.
- Primary Reviewer's Form (PDF) This form is normally used in the review process of “Yes" disclosures, conducted by Advisory Committees on Investigator's Conflicts of Interest. The Primary Reviewer will normally speak to the investigator when completing this form and the investigator will receive a copy after the committee meeting.
- Trainee Monitoring Guideline When students or trainees will be on the project team, this guideline is usually incorporated into the Committee's management plan for a conflict of interest affecting the project.

A Project Specific Disclosure form must be filed to disclose, initially, any significant financial interest that could reasonably appear to be affected by the design, conduct or reporting of covered research. "Covered research" on the Charles River Campus (CRC) includes all externally funded research and all human subjects research. "Covered research" at the Boston University Medical Center includes all research. This same form is also used for "supplemental disclosure" to report any "material change" in a previously filed disclosure.
