after they have been downloaded and completed.
Please note that you must have the application Adobe Acrobat Reader in order to download
and view the following forms. Acrobat Reader is a free product which allows you to view, navigate, and
print Portable Document Format (PDF) files across many different platforms. Select your operating
system once you have gone to the
Adobe site to download the application.
- Registration Form
Each School has different advising procedures. If you are unsure of your School's requirements, please contact
your advising office for more information.
No forms will be processed without the appropriate advising
signatures. Please note the University's Legal Notice regarding class registration.
- Class Adjustment Form
Classes may be added on a space available basis for approximately two weeks after the start of the
semester. (Please see important dates
for specific deadlines.)
Some Schools require an advisor's signature for any changes to your schedule. If you are unsure of your School's
requirements, please contact your advising office for more information.
- Transcript Form
Transcripts cost $5.00 per copy and will not be released without payment. Transcripts will also not be
released if you have not fulfilled your financial obligation to the University. Please note that current semester grades
are not posted to transcript until one week after final exams.
- Personal Data Update Form
Use this form only if you do not have access to update this information on the Student Link.
Complete the information and mail it to the address above.
- Cross Registration Information and Forms
Cross-registration is a formal agreement between Boston University and several local colleges and
universities whereby BU students can attend courses at other institutions, and students from other
institutions can attend courses at BU. The primary
schools in the cross-registration consortium are Boston College, Brandeis University, Tufts University and Hebrew College.
