What's involved for patients?

TO START: We ask patients to complete a short enrollment form, sign a consent form/medical record release, and mail these to the registry. You can give them an enrollment packet, which contains these forms and other information about the registry. To request enrollment packets for your office, please contact us by calling 1 (800) 231-3769 or send an email to registry@slone.bu.edu.

Patients can also enroll via the internet by visiting the relevant registry website:

NEXT: Soon after enrollment, we send patients a baseline questionnaire that inquires about disease symptoms, treatment, and quality of life measures.

THEN: Every six months, we send follow-up questionnaires to ascertain current treatment and assess quality of life.

Patients who prefer may complete their questionnaires at our website.
