How does the registry work?

TO ENROLL: Just complete a short enrollment form that you may obtain from your doctor or by calling toll-free 1 (800) 231-3769. You can also enroll on this website by clicking here. We will also ask you to sign a consent form that will give us permission to obtain your medical records from the doctor or doctors who are taking care of you.

NEXT: Once we receive your enrollment form, we will send you a questionnaire that asks about treatments you may be receiving for your myeloma, symptoms you may be experiencing, and any problems that you may be having with daily activities.

THEN: On an ongoing basis, you will receive similar questionnaires every six months. If you prefer, you may complete your questionnaires at our website.
