How does the registry work?

TO ENROLL: Just complete a short enrollment form that you may obtain from your doctor or by calling toll-free 1 (800) 231-3769. You can also enroll on this website by clicking here. We will also ask you to sign a consent form that will give us permission to obtain your medical records from the doctor or doctors who are taking care of you.

NEXT: Once we receive your enrollment form, we will send you a questionnaire that asks about treatments you may be receiving for your myelodysplastic syndrome (MDS), symptoms you may be experiencing, and any problems that you may be having with daily activities.

THEN: On an ongoing basis, you will receive similar questionnaires every six months. If you prefer, you may complete your questionnaires at our website.
