How do I enroll?

There are 3 ways to enroll in the registry.

• Enroll on the web. Complete the online enrollment form by clicking here.
  
  
• Call the registry, toll-free, at 1 (800) 231-3769 to request an enrollment packet.
  You may also email the registry at mdsinfo@slone.bu.edu.
  
  
• Ask your doctor for a PRS : MDS Enrollment Packet. Then complete the enrollment form in the packet and mail it back to us, in the postage-paid envelope enclosed for your convenience.
