Department Chair's Evaluation

The Chair is to review the application to ensure that all questions addressed to the candidate on the Faculty Request form are answered and all supporting material is attached.

Following is a list of data elements that must be completed:

  1. Name
    Use the format last, first, middle initial.
  2. School/Department
    List the academic unit and department name. Commonly used acronyms are acceptable.
  3. Academic Year of LOA/SAB
    Indicate the academic year for which the LOA/SAB is proposed.
  4. Chair's Evaluation
    In the space allotted on the form, an evaluation of the scholarly merits of the proposal and its contribution to the department should be provided by the department Chair.
  5. Preliminary Recommendation to Staff Candidate's Courses/Duties
    Except under unusual circumstances, other department members are expected to teach those courses that will not be canceled or postponed, and to assume any necessary administrative duties. In the unusual circumstance where additional resources are being requested, describe the replacement strategy and justify the funding request in the appropriate section.
  6. Signature
    The Chair is to sign on this form, as well as on the appropriate line of the Faculty Request form, being sure to also check "concur" or "dissent" for both the LOA/SAB request as well as for the tenure deferral request.
