BUPC Facility Requests

This form is for all building related requests, including but not limited to custodial assistance, maintenance, equipment or furniture moving, removal, and/or installation, deliveries that involve the loading dock, electrical issues, HVAC, plumbing requests, etc. for labs and/or offices on Photonics Center floors (Basement, 5^th, 6^th, 7^th, 8^th, and 9^th).

PLEASE NOTE: If this is an emergency request (i.e. flooding, chemical spill, smoke, compressed gas/air leak, temperature issues, etc.) please call 617-353-2105 and report this incident to the emergency call line. Please be sure to include your name, the building location, the room location or nearest to if it is in the hallway, and a brief description of what the emergency is. Be as descriptive as possible and then fill out this form.

- an urgent, non-emergency request - 48 hour turn-around
- a standard, non-emergency request - 1 week turn-around
- a shared laboratory equipment problem (be sure to specify in the Description of Request box below, which lab, OPF, PML, or FTF, which piece of equipment, and detail out the problem)
- a new equipment delivery request - minimum of 1 week advance notice required
- an equipment installation request
