Items Required by OSP-MED to Establish a Source Account
For all new or renewal prime awards, incoming subawards, and pre-award accounts for prime awards, OSP-MED will assign a source number and issue it to the department upon review and approval of:
- Notice of Award from sponsor
- Budget (by object code) with budget justification
- If budget not received, OSP-MED applies cuts, usually to the supply (924) and/or travel (918) lines proposed in the application. See Object Code Budget.
- Note that a modified Scope of Work may be required if incoming subaward budget warrants.
- Subaward contact information sheet (for outgoing subaward and for incoming subaward from another institution)
- Outgoing subawards’ signed letters of intent, scope of work, budgets, biosketches, if not previously provided at application stage or if modified since application
- Letter of Intent for research services performed by consultant(s) [check with your research administrator for template]
- IRB approval information for all protocols (new, continuing, and exempt) conducted under the award
- For each new protocol starting this year, provide an approval letter.
- For each protocol continuing from the previous year, provide the protocol number.
- For each protocol that IRB has considered exempt, provide exemption number.
- Human subjects letter and individual certifications
- IACUC approval information for all protocols (new and continuing) conducted under the award
- For each new protocol starting this year, provide an approval letter.
- For each protocol continuing from the previous year, provide the protocol number.
- Radioisotopes/rDNA/IBC approval documentation by Research Safety, overseen by the Office of Research Compliance
- Project-specific Disclosure (Conflict of Interest) form, if not previously submitted, for any individuals who will be designing, conducting, or reporting on covered research, on the Research Compliance site
- University’s resolution of the conflict of interest, if there is one.
- Memorandum of Understanding for each researcher with a joint appointment at both BU and VA, who is committing salaried effort to the project
