  • IBC Application:
    • The IBC Application for work with Recombinant DNA/Biohazards (Biological Use Authorization) is available through RIMS.  
  • IBC Protocol Amendment:
    • For all 2010 and 2011 IBC Protocols that are COMPLETELY UPDATED IN RIMS, complete the Amendment in RIMS (see instructions here) and click “confirm the submission” to send it to the IBC Office for amendment evaluation.
    • For 2009 IBC Protocols and those 2010 and 2011 Protocols that are not completely updated in RIMS, complete the paper IBC Protocol Amendment Formand send to ibc@bu.edu, by fax to 617-638-4226, or through inter-office mail to 85 E. Newton St – Fuller (M) 810.
  • IBC Protocol Closure Form:
    • To close a current IBC protocol upon completion of a project, please complete this form and send to ibc@bu.edu.  
  • New IBC Investigator CV format:
  • The Initial Health Questionnaire for the Research Occupational Health Program is available through the ROHP website.
  • The Authorization for Services for Hepatitis B Vaccine Series form is also available through the ROHP website.
