Financial Conflicts of Interest – MS Word/Adobe Forms (paper process)

How to submit completed forms:

Please send completed forms to coi@bu.edu or via fax at 617-414-4738  no later than final submission of the research application. You may sign the forms by typing in your name or pasting in a digital picture of your signature within the signature box. If you do so, ensure the form is sent from your private BU, BMC, or other home institution email address to confirm identity.

Note: You are highly encouraged to save your disclosure forms on your computer so that you can refer back and edit them for future disclosures, rather than start from scratch.

Financial Interest Disclosure Forms

These forms must be filled out if you are now or will potentially be responsible for the design, conduct, or reporting of research activities at Boston University and/or Boston Medical Center. This includes, at minimum, all those listed as senior/key personnel on the grant application, where applicable. Please be advised that you may be requested to provide additional information based on the information provided in these forms.

FIND 1 and FIND 2 forms are needed for:
i. All research administered at BMC (regardless of funding)
ii. BU programs and research sponsored by any of the following organizations/agencies:

Administration for Children and Families
Administration on Aging
Alliance for Lupus Research
American Cancer Society
American Heart Association
American Lung Association
Arthritis Foundation
CurePSP
Susan G. Komen Foundation
Juvenile Diabetes Research Foundation
Lupus Foundation of America
Office of Global Affairs
Office of the Assistant Secretary for Health
Office of the Assistant Secretary for Preparedness and Response
Public Health Service: AHRQ, ATSDR, CDC, FDA, HRSA, IHS, NIH, and SAMHSA (PHS), except Phase I SBIR and STTR

FIND 1: For All BMC and BU PHS-Funded Research

This form is required by those responsible for the design, conduct, or reporting of research activities, including, at minimum, those listed as senior/key personnel on the grant applicationwhere applicable—prior to the final submission of the research program application, within 30 days of a change to any reported financial interests, and annually during the period of the award.

FIND 2: Addendum for All BMC and BU PHS-Funded Research

This form is a companion to the FIND 1 form and is required whenever a “yes” answer is provided on the FIND 1. Information requested includes details about financial interests related to one’s institutional responsibilities, as well as directed questions about the relatedness of disclosed financial interests to the specific research program associated with the form.

NON-FIND 1 and NON-FIND 2 forms are needed for:

BU research not included in the above list, including unfunded and departmentally funded human subjects research.

NON-FIND 1: For Non-PHS-Funded Research at BU Only (Not BMC)

This form is required by all those responsible for the design, conduct, or reporting of research activities, including, at minimum, all those listed as senior/key personnel on the grant application where applicable prior to the final submission of the research program application and whenever there is a material change to one’s “significant financial interests” (as defined by institutional policy).

This form is a companion to the NON-FIND 1 form (above) and is required whenever a “yes” answer is provided on the NON-FIND 1. Requested information includes details about the “significant financial interests” indicated in the NON-FIND 1, and their relation to the specific research project associated with the disclosure.
