Home Care

Home care is a major part of our treatment plan. Our multidisciplinary team recommends exercise regimens, teaches stress management techniques and develops plans for limiting sodium and fluid intake. Our nurses are in close contact with patients by telephone on a frequent basis to monitor symptoms and assess medication compliance and fluid balance. Sometimes these simple steps can pre-empt emergency room visits and hospitalizations.
