Taking a Leave of Absence (Paid and Unpaid: Medical, Personal, Military, Sabbatical)

To initiate a leave of absence request, the employee must complete the appropriate leave request form:

Staff
Faculty

Certain coverages may continue during your unpaid leave, as described below. If you wish to discontinue or suspend your membership in any of these plans, you will need to complete the necessary forms, available from Human Resources. Re-enrollment in these plans is possible when you return from a leave of absence, as long as you contact Human Resources and enroll within 30 days from the date you return.

While you are on leave, you must notify Human Resources of any changes in your return-to-work arrangements. Please also refer to Section 300 of the Employee Handbook regarding leaves of absence.

If you or a dependent needs personal help, you may contact BU’s Faculty & Staff Assistance office for confidential counseling and referral services.
