Group Supplemental Life Insurance Plan
If you are classified by the University as a regular employee, work a full-time schedule, and have an appointment of nine months’ or more duration, you and your eligible family members are eligible to participate in the Group Supplemental Life Insurance Plan on your first day of active employment.
Reducing to a Part-Time Schedule — If you become ineligible, you lose coverage. Employees whose schedule changes to less than the eligibility requirement will not be able to continue to participate in the Group Supplemental Life Insurance Plan. If you no longer meet the eligibility requirement, your coverage will terminate at the end of the month in which your percentage time or length of assignment changes. In order to rejoin the plan, you will need to meet the eligibility requirement at the time you would like to participate.
Your spouse must not be hospitalized, confined at home under the care of a doctor, receiving disability benefits, or unable to perform the normal daily activities of a person of the same age and sex.
Your child(ren) must be at least 15 days old and less than 19 years old. Dependents must be unmarried to be considered eligible.
