Survey to Assess the Current State and Impact of Quality Improvement Activities in U.S. Hospitals

Under a $326,195 grant from the Commonwealth Fund, the HPRP surveyed a sample of U.S. hospitals regarding the current state and impact of quality improvement (QI) activities across the hospital industry.  Partnering with the HPRP on this project was the Health Research and Educational Trust (HRET), the research arm of the American Hospital Association.  Two complementary surveys were developed and administered in 2006: (1) The Quality Improvement Activities Survey (QAS), which gathered information about the nature of QI activities undertaken, and their impacts on organizational structure and processes in U.S. hospitals; and (2) The Clinicians’ Perceptions of Quality Survey (CPS), which elicited assessments from front-line nurses and physicians in each QAS-sampled hospital regarding the quality of patient care in their organizations and the structural and process changes taking place there.  Survey responses were received from 470 hospital chief quality officers and 5,383 clinicians in 566 hospitals.  The survey data have been analyzed regarding the nature, breadth, and perceived value of the QI activities undertaken by hospitals in the interval since the 2001 release of the Institute of Medicine’s landmark report, Crossing the Quality Chasm.  Survey data also have been analyzed in conjunction with hospital performance measures that are routinely collected by the Centers for Medicare and Medicaid Services (CMS), and the study has yielded practical lessons about best practices that may be adopted by hospitals to improve quality of care.  HPRP investigators included: Alan B. Cohen and Joseph D. Restuccia (co-principal investigators), Michael Shwartz (principal statistician and co-investigator), Sally K. Holmes (survey design), and Jennifer E. Drake (survey design and analysis).
