The Functional Status Index (FSI) was developed as a comprehensive ADL assessment for adults living in the community. The 18 items include the following domains gross mobility, personal care, social/role activities, hand activities and home chores. Items are scored to provides scores in three areas: dependence, difficulty and pain.
