Quality Assurance Program Required Documents

Students selected as part of BU’s QA (Quality Assurance) sample are required to provide, at a minimum, income verification documentation. Please note income verification documentation must be provided through one of the specified methods detailed below. Copies of tax returns, which may have been previously submitted to BU Financial Assistance, cannot be used for QA income verification purposes.

In addition to providing income verification you may be asked to provide one of the following:

  • Proof of identity
  • A signed Statement of Educational Purpose
  • Verification of your household size and number of household members in college
  • Food Stamp receipt verification
  • Verification of child support paid by you or your parent

Students selected as part of the QA sample will be contacted individually by email with their personalized requirements.

The required information is academic year specific.

2015-2016 QA Required Information

Student Income Verification – required of all selected students

2014 income verification information must be provided using one of the following options.

  • IRS Data Retrieval Tool
    The IRS Data Retrieval Tool is available through the FAFSA online application process. It allows you to transfer your income data directly from the IRS to your FAFSA. You must have a 2014 US Income Tax Return on file with the IRS to use this option.

    BU highly recommends you use this option, if possible. Watch this Data Retrieval Tool Video for more information about how to use it. Note: Some tax filers are not eligible to use the Data Retrieval Tool. If you are unable to use the Data Retrieval Tool and are required to file a 2014 Federal Income Tax Return, you must submit a Federal Tax Transcript instead (see below).

    If you have not already done so, you must create and log in with an FSA ID before using the Data Retrieval Tool.

  • 2014 IRS Federal Tax Transcript*
    A tax transcript must be requested from the United States Internal Revenue Service (IRS). A tax transcript is an exact record of the information provided on the US Income Tax Return you filed with the IRS. Information on how to request an IRS Federal Tax Transcript can be found on the IRS website. A Federal Tax Return may not be submitted in place of a Federal Tax Transcript. Please note: Currently the IRS can only provide a paper transcript which, if requested, is sent to the taxpayer by mail. Because getting a paper transcript by mail can take 5 to 10 calendar days, we will extend our deadline if you contact us and advise us that you have requested a paper transcript. 
  • *If you have filed a revised US Federal Tax Return (1040-X) since you originally filed your taxes you must submit a 2014 tax transcript and a 2014 tax account transcript. Both documents are required to fully verify your income information.
  • Non-Tax Filer Income Verification Statement
    If you were not required to file a tax return for the 2014 tax year you will need to complete the Student Income Verification Form:
    Student Income Verification Form – Dependent Students
    Student Income Verification Form – Independent Students

Parent Income Verification – required of all dependent selected students

If a student’s biological or adoptive parents are married, income verification is required for both parents. If student’s biological or adoptive parents are separated, income verification is required from the custodial parent (and the custodial parent’s spouse).
