Boston Breathes: Boston Breathes is an interactive web-site to promote patient self-management and primary-care/-specialist teamwork for children with asthma.
 
The Study: We will be evaluating Boston Breathes in a randomized clinical trial. The study will compare asthma care and outcomes for children in three groups: 1) usual asthma care; 2) Boston Breathes – primary care MD and child/family only; and 3) Boston Breathes – primary care MD, asthma specialist, and child/family. Kids who are 9 -17 years old AND have persistent asthma are eligible. We hope to show that Boston Breathes can decreased asthma symptoms, improved controller medication adherence, and improve asthma quality of life.
Expectations for participants: Each child in the Boston Breathes group will agree to visit the web site at least weekly. Participating clinicians will review patient-entered information at least weekly (if new information has been entered). Estimated clinician time per week will not exceed 2-3 minutes per patient.

Benefit for the Patient

Benefit for the clinicians:

  • Interactive asthma education and self-management promotion (symptom recognition, peak flow monitoring, medication adherence, and trigger avoidance are the primary goals)
  • Easy to use discussion board to update primary care clinician
  • Regular and accurate clinical information from the patient in the home (peak flow and symptom scores), presented in an “at-a-glance” graphical format
  • Easy to use discussion board for patient communication.
Activities online for patient: Activities online for the clinicians:
  • Educational modules
  • Reporting peak-flow values via web entry
  • Reporting symptom scores online
  • Communicating with PCP, specialist, and peers online via asynchronous discussion groups
  • Viewing graphical output of peak-flows and symptoms.
  • PCP, or nurse, will log in about once weekly to review patient entered information
  • Send a brief message to the patient/family via the discussion board
  • Estimated time per week per patient: 5 minutes.
This method of asthma management was developed and implemented in Germany in 2000 and led to decreased utilization and cost of health services. It has been accepted and reimbursed by German health insurers as a bona-fide health education and disease monitoring system.
