How to Digitally Sign a Form

Transfer Credit Form
Once complete, all forms can be emailed to enggrad@bu.edu with accompanying documents. Please be aware that only electronic forms will be accepted.

LEAP to Master’s Matriculation Form
LEAP Students who will have completed all of their foundational courses in the current semester should submit this form to transition into their Master’s program for the following semester. Students who have no more than one remaining foundational course to complete may also submit this form, provided they have received approval from their advisor. Students who wish to be considered for a tuition scholarship must provide a faculty recommendation.

Program Change Form
Once complete, forms can be emailed to enggrad@bu.edu. Please be aware that only electronic forms will be accepted and requests are not guaranteed.

Withdrawal Form
Leave of Absence Form

Boston University graduate students interested in permanently discontinue their academic program or taking a leave of absence from their academic program must submit the Withdrawal/Leave of Absence Form to enggrad@bu.edu

Add/Drop Form

Classes may be added on a ‘space available’ basis for approximately two weeks after the start of the semester; see Semester Dates for specific deadlines. Once complete, please email form to enggrad@bu.edu with any accompanying documents. Please be aware that only electronic forms will be accepted.

Registration Form

Once complete, all forms can be emailed to enggrad@bu.edu. Please be aware that only electronic forms will be accepted. Registration Forms will not be processed without the appropriate advisor’s signature. Please note the University’s Legal Notice regarding class registration.
