ENG Room Access Forms

These are the old forms that are being phased out and will only be used in special cases.
24 Cummington St. is a special case. Use this form for 24 Cummington, including the BME basement labs - LSEB B-03 A and B, B04, BME Computational Simulation Facility. Use the link for Biomedical Engineering below.

Do not use this form unless you have been directed by the Zaius site in the FAQ. You must use Zaius to request room access for MOST spaces in Engineering. Read the FAQ in Zaius to determine if you need to use Zaius to request rooms in Zaius.
Zaius site: http://www.bu.edu/eng/zaius/

Please make sure your card does not work before filling out this form. In many cases, access is processed before the semester begins. Usually, this means students who are pre-registered. Please select the Dept. or Center that controls the space you need access to.
(You will need a Kerberos Password. If you do not have one, call the Dept. to process a paper form.)

After you have submitted a room access form, you will receive an e-mail confirmation after your request has been processed . This may take up to a week. (In September and January, it may take up to 2 weeks.)

After you have received the e-mail, check to make sure your card works in, at least, one of the readers/rooms you requested. You must run the card through the reader many times. Your card may not work because of the replacement/issue code that is added to your ID card by the Terrier Card Office (each time you get a new card the replacement/issue code increases by 1). The changed replacement/issue code must be manually changed in the room access system. If your card does not work, respond to the e-mail stating your card is not working. You should receive a response in 24 hours (48 hours in September and January). After you get a response, you will need to check your card again to make sure it works. Also, if the card does not work, another option for ECE people (only - at this time) is to go to Room 324 (M-F, 9-5pm), and ask that your card be run through the reader on Rosemarie's desk. This swipe will generate an e-mail indicating the correct replacement/issue code to the College Room Access Coordinator. It will be changed in 24 hours (48 in September and January). You may want to use this option, if your card has been replaced more than once.+++++++++++++++++++++++++++++++++++++++++++++++++++++++++++++++++++++++++++++

Your request is sent to the Dept. Room Access Coordinator who will seek approval for each room requested and the expiration date. Several people may have to review this request and it may take up to a week to process. The Dept. Room Access Coordinator may be able to give you a better idea how long it will take but most requests are done in 2 days (4 days in September and January). After the form has been processed, access will be granted by using your Boston University Terrier Card.
