Pre-Matriculation Health Requirements
All incoming students and residents are required to complete pre-matriculation health requirements and submit required paperwork via Patient Connect prior the start of Orientation.
- A physical exam performed by your primary care physician
- An Immunization & Physical Examination Report completed by your physician and submitted to Student Health Services via Patient Connect
It is highly recommended that students make an appointment for a physical exam as soon as possible after they have received their acceptance notification. You may be required to complete a series of inoculations in the months leading up to Orientation in order to be compliant with University and Commonwealth of Massachusetts regulations.

Please do not contact Student Affairs or Admissions regarding your medical records or send us any forms. We are not allowed to see your protected health information, nor do we have access to your medical record.

Please log on to the Student Link and click on the “Personal” tab, then click “Compliance”. You can review your records, see your current compliance status, and review what documentation you may have to submit via Patient Connect.
