Pre-Matriculation Health Requirements
All incoming students (pre-doc & post-doc) must have a physical exam by their primary care physician and submit a Health History and Physical Examination Report (MED Campus) prior to Orientation.
It is highly recommended that students make an appointment for a physical exam after they have received their acceptance notification.
It is important to schedule your appointment as soon as possible as you may be required to complete a series of inoculations months prior to Orientation to comply with University and Massachusetts regulations.
Students should also review the Student Health Services Welcome Letter & Instructions for more specific instructions and important GSDM inoculation information.
Deadline to submit this form is:
- June 1, 2012 – Post-doctoral students
- July 1, 2012 – DMD and Advanced Standing Students
- August 1, 2012 – September start post-doctoral students
Because this form will contain your confidential medical information, it must be submitted directly to Student Health Services. Please DO NOT submit the form to GSDM.
