The following Student Organization/Class forms are now available ONLINE at http://gsdm.bumc.bu.edu/portal/ under the “Student Affairs” tab.

• Student Event Request Form
• Financial Activity Request Form
• Student Mass Communication Request Form
• Create New Student Organization Form
• Student Org Registration Form and Membership Roster

In our continuing effort to be more green, we have moved away from paper forms to this online system.  We also feel that this will increase efficiency for both our student organizations and the Student Affairs Office, allowing us to better serve you, our students.
