Letter of Offer
The onboarding process at Boston University begins with a signed offer of employment. This letter provides the basis for an individual’s specific contract with Boston University. Once you have been offered a position at GSDM, your Chair will work with the Dean of the School to tender a conditional employment offer. This document will detail baseline information about your employment with Boston University including your status (full-time, part-time, volunteer, affiliate), the number of days you are expected to work (employment effort), your salary and benefits, duties and responsibilities, and also contains information related to the specific type of faculty contract under which you have been employed. This letter will be communicated to you by your Chair, but will only be considered fully executed when countersigned by the Dean. The Dean will countersign employment letters only upon receipt of the GSDM Heath Screening packet.
GSDM Immunization Process and Health Screening Packet
As a health-care facility, GSDM must comply with certain regulations and protocols as set forth by the Centers for Disease Control, the Commonwealth of Massachusetts Department of Public Health, and the Boston University Medical Center Office of Occupational & Environmental Medicine (OEM). All employees must provide documented proof of immunization for: Hepatitis B, MMR (Measles, Mumps, Rubella), Varicella-zoster live virus vaccine, Adacel or TDAP (tetanus, diphtheria, acellular pertussis), Tuberculosis, Influenza.
If you lack sufficient documentation to validate these required immunizations, you must participate in an updated immunizations review process with either your primary care physician or OEM. OEM will open an employee patient record on your behalf and provide you with a Certificate of Clearance indicating that you have met the required immunization protocols required as a condition of employment at Boston University Henry M. Goldman School of Dental Medicine.
You may download the immunization packet either as a Microsoft Word or Adobe Acrobat file. Please read and complete the enclosed forms, seal in an envelope and return marked <CONFIDENTIAL> to the Office of the Dean. Your confidential packet will remain sealed and be hand delivered to the Boston University Medical Center Office of Occupational & Environmental Medicine which will evaluate your status and contact you for additional information (if required). As a new employee, should you require inoculations or booster shots, these will be administered through OEM at no cost to you. Regrettably, we cannot reimburse you should you choose to pursue treatment through your own physician. Once you have submitted your packet to the Office of the Dean, you will receive a fully executed copy of your offer letter and may commence employment.
