Synesthesia Questionnaire

In order to ensure total anonymity, we will not collect any personal identifiers during this questionnaire, including your name and email address. Instead, you will generate a unique, random survey code when you click the "Begin" button.

The survey code serves as your unique, anonymous identifier in our system. In addition, the survey code will allow you to stop the questionnaire and return at any time, without loss of previously entered information. All you will need to do to continue the questionnaire is paste your survey code into the box below.

Please note: Since the survey code is unique and anonymous, no one in the Synesthesia Project can access it. If you lose your survey code, we will not be able to retrieve it for you. Therefore, it is very important for you to copy-paste your code into a document (such as a Word .doc or .txt) to save it or for you to bookmark the next page (not this page) in your browser.
