NOTE: IF YOU SUBMITTED A SHORTER ALTERNATE YEAR REVIEW LAST YEAR, YOU MUST USE THE FULL FORM THIS YEAR. CHOOSE FULL REVIEW OR ALTERNATE REVIEW BELOW.

Following are the salient details:

  • The submission deadline is 1-31-2010. Late submissions will not be reviewed.
  • The period under review will cover the calendar year (Jan 1st 2009- Dec 31st 2009).
  • A key component is to think about your goals for 2010, and how you plan to reach them.
  • Do you plan to make any changes in your teaching roles for next academic year (July 2010- June 2011)? Think about this too, as you will be asked to indicate preferences for the coming year.
  • Update your CV- use the BU form link above if you wish.
  • Schedule a meeting in January with your supervisor to review your FPR submission once complete- please get this meeting in your calendars now.
  • Please note that participating in this process is required of anyone employed by the BU Department of Family Medicine.
Specifics:

Step 1: Complete evaluations

  • 5 peers that are assigned to you on the New Innovations system. Click New Innovations system to enter. THIS IS READY FOR USE now. DEADLINE December 30, 2009.
  • • Please review your supervisor and other managers of clinical sites . This is ready now on New Innovations. You will need to select your supervisor and manager from each area you work in from a dropdown menu

Step 2: Make an appointment with your FEBRUARY supervisor to discuss your goals and teaching plans.

Step 3: Prepare Submission DEADLINE JANUARY 31, 2010

  • Update your Boston University CV. Email a copy to Janice Robbins and Florence LaForest.
  • Logon to view FPR webform and follow instructions to complete.
  • You need to select a username and a password once to login to the form by selecting Register link on the link page. By default your username is your first initial and full lastname. For example, John Smith username is jsmith.

Step 4 : Florence will collect into a folder and provide a copy for you to take to the meeting with your supervisor and one to the review committee that includes:

  • Pre-doctoral Evaluations (clerkship, clinical precepting, small groups, online moderation, ICM I & II and Electives)
  • Residency Evaluations (precepting outpatient, attending in hospital, Monday and Wednesday Conferences, Grand Rounds and others as applicable)
  • Peer Evaluations from New Innovations system
  • Staff Evaluations if available
  • Patient Evaluations if available
  • Clinical productivity and quality (your Medical Director will provide these to us)

Step 5: Assemble additional documentation as needed (optional for full review only), including sample teaching materials, letters of recommendation, and any presentations, abstracts, manuscripts, abstracts of grant applications, new curriculum, or other materials produced that might be of interest to the FRC. Submit these to Florence ON DOWLING 5.

Step 6: Meet with your direct supervisor to review your portfolio, goals for the coming year, and your achievement of goals from last year. Your supervisor will put an evaluation and comments online after reviewing your goals with you. The Faculty review committee will meet and review each faculty's submission and return comments electronically.

Step 7: Once you receive your report from the committee, we suggest that you meet with your direct supervisor again to review your FRC report.
