Learning Objectives:  

After participating in this activity, clinicians should be better able to:

1. Identify at least one barrier in their practice to the effective treatment of their patients with type 2 diabetes

2. Establish a plan to implement improvements in their office-wide systems for the case management of patients with type 2 diabetes

3. Outline a systematic approach in their practice to the diagnosis and treatment of type 2 diabetes that leads to improved long-term outcomes
