Tenure and Tenure-track, Visiting, Adjunct, Lecturer, Research
- Chair’s Memo of Recommendation
- Academic Appointment Form*
- Biographical Data Form
- Three Letters of Recommendation
- Current Vita (no older than 3 months)
- Letter of application from candidate
- Publication Sample
- Affirmative Action Report (Full-time only)
* There are now specific forms that differ from the Academic Appointment Form for part-time, research, and secondary appointments. See the "Appointments (other)" section of the Provost forms website.
Research Fellow and Visiting Scholar (non-compensated)
- Sponsoring Faculty Member’s Memo of Recommendation with chair's endorsement
- Academic Appointment Form
- Current Vita (no older than 3 months)
Part-time Reappointment
This includes RsFel, VsSch and FT Research Faculty.* Faculty returning to Boston University after a period of more than 3 years will require the submission of initial appointment forms.

Change of Title/Change of Status

* Use a Change of Title/Change of Status form if you are reappointing and changing title and/or salary.
