Required Paperwork for Faculty Appointments

These forms are also available in the Forms Library on the website of the Office of the Provost in the event of problems with links below. The Provost’s Office website also includes information on the final level of approval needed for appointments.

Tenure-track, Clinical, and of the Practice Faculty

  • Recommendation for Academic Appointment Form (helpful guidance for completing this form is available on the Provost’s website)
  • Chair’s Memo of Recommendation (faculty vote required)
  • Three External Letters of Recommendation
  • Current Vita (Please upload cv no more than 3 months old)
  • Letter of application from candidate
  • Signed Intellectual Property Agreement (Must accompany initial appointment paperwork for full-time hires)
  • Affirmative Action Report (Online for tenured/tenure-track faculty)
  • Cover Page for Board of Trustees (only required for unmodified professorial ranks)

Note: New appointments at the ranks of full Professor and Associate Professor require a complete tenure review; please contact Alex Lundsted in the Office of Faculty Actions about the requirements for such appointments. New appointments at the ranks of Associate Professor of the Practice and full Professor of the Practice also require a review similar to the tenure review; again, contact Faculty Actions regarding the requirements for appointments at these ranks.

Full-time Lecturers, Adjunct Professorial Faculty, Visiting Professorial Faculty

  • Chair’s Memo of Recommendation (faculty vote for all full-time and all professorial-rank appointments)
  • Three External Letters of Recommendation
  • Current Vita (Please upload cv no more than 3 months old)
  • Signed Intellectual Property Agreement (Must accompany initial appointment paperwork for full-time hires)
  • Affirmative Action Report (Online for full-time Lecturer hires; not required for adjunct and visiting faculty)
  • Provost’s approval checklist for adjunct professorial faculty with one-semester appointments

Part-time Lecturers

  • Recommendation for Academic Appointment Form

If courses assigned to PT lecturers are cancelled due to low enrollment, please be sure to notify the Faculty Actions Office.

    Visiting Researchers (non-compensated)

    Part-time Faculty, Adjunct Professorial Faculty

    Part-time faculty returning to Boston University after a period of more than 3 years will require new initial appointment paperwork.

      Visiting Researchers (non-compensated)

      • Change of Title/Change of Status Form* (includes space for Chair’s/Director’s rationale)
      • Current Vita (Please upload cv no more than 3 months old)
      • If the change is for a reduction in effort, please include a statement from the individual acknowledging that the reduction in effort may impact their benefits

      * Use a Change of Title/Change of Status form if you are changing title and/or salary ONLY. Requests to change academic researchers to faculty ranks are considered to new appointments, not changes of status or promotions.

      • Endorsement from Chair/Director where the faculty member holds a primary appointment

      Sabbatical/Academic Leave of Absence

      • Sabbatical/Academic Leave of Absence Request form

      • Detailed plan for the sabbatical/leave of absence period (1-2 pages)

      Please note CAS policies related to sabbatical/academic leave of absence and fellowship salary gap support. Copies of applications for external fellowships or grants are NOT required.

      Non-academic Leave (Medical/Childbirth/Other Personal Leave)

      • Non-Academic Leave of Absence Form
      • Medical Certification Form, to be completed by physician and returned directly to HR (Medical leave requests only)

      Non-academic leaves for medical reasons and for childbirth are normally with full compensation for full-time faculty (up to six months for medical leave); other non-academic leaves are normally uncompensated.
