Please fill out the appropriate LASC form(s) from the list below.

Security Access Application Form (PDF)

Access to the animal facility is initiated with the submission of a completed Security Access Application form.  The Employee Section must be complete with all pertinent information.  Special attention is directed to assure the application contains the following: protocol #,  dates of trainings and Medical Surveillance, Principal Investigator signature and applicant signature.  In order for access to be granted the applicant must be listed on the specific IACUC protocol. Completed Security Access Applications can be submitted to the LASC via: fax: (617) 638-4055, e-mail: lasc@bu.edu, or mail delivery/personal delivery to 700 Albany St, W707.

Authorization Form for Animal Housing and Special Services Costs (PDF)

This form must be filled out and forwarded to the LASC in W-707 by inter-office mail or faxed to the LASC at (617) 638-4055.

Transgenic Mice/ Genetically unique animals

PIs who are shipping out mice to collaborators,  should contact the Office of Technology Development for guidance regarding any additional forms that need to be submitted. •  Transport/Transfer Request (PDF)

This form must be filled out and filed with the LASC in order to transport or transfer animals from one housing room to another within the LASC or from one protocol to another.

• Metabolic Cage Request Form (Word)

Submit a request to the LASC Office (W-707) at least two business days in advance of your requested start date. A metabolic cage scheduling calendar can be found in the LASC Office if you want to confirm availability before signing up.

• Occupational Health Questionnaire (select Annual Health Status Questionnaire on that page)

This form must be filled out and submitted to the Research Occupational Health Program (NOT LASC). See below for submission details. You must also submit an OSHA Respiratory Medical Evaluation Questionnaire (see below).

This form must be filled out and submitted to the Research Occupational Health Program in the Evans Building (E), 72 E. Concord St., 8th Floor. Tel: 617-638-ROHP

• Special Services Purchase Order Form (PDF)

Use this form to purchase medications or services from the LASC. This form can be downloaded, filled out on the computer, and faxed to LASC.

Revoking Access to the Animal Facility

• Termination of LASC Facility Access /Revocation-Access Forms (Word)

This form should be submitted by fax at (617) 638-4055 to the LASC Office by the Principal Investigator whenever an employee/student is terminated from a project or lab and no longer needs access to the LASC facility.

It is the responsibility of the Principal Investigator to ensure that the LASC Business Office is notified whenever an employee’s access to the LASC facilities should be revoked. Thank you for your cooperation with this very important security issue.
