Please use the forms listed below for work related to the LACF.
- LACF Animal Housing Authorization Form (pdf)
This form must be filled out and forwarded to the LACF at 5 Cummington Street by inter-office mail or faxed to the LACF at (617)353-4527.
- Animal Requisition Form (Word)
This is to be used in conjunction with an open purchase order, or to let the LACF know you are putting in an animal order through the online requisitioning system.
- Special Care Request Form (Word)
This form is to be used to request services that are not part of routine services. (such as food restriction, water restriction, delayed food or water).
- Security Access Application Form (Word)
Access to the animal facility is initiated with the submission of a completed Security Access Application form. The Employee Section must be complete with all pertinent information. Special attention is directed to assure the application contains the following: protocol #, dates of trainings and Medical Surveillance, Principal Investigator signature and applicant signature. In order for access to be granted the applicant must be listed on the specific IACUC protocol. Completed Security Access Applications can be submitted to the LACF at 5 Cummington Street by inter-office mail or faxed to the LACF at (617)353-4527.
