Medical Release – Submit medical history and current immunization records; required of all students enrolling at the Academy.
Emergency Card - Provide contact information in the case of an emergency.
Appropriate Usage Policy - Agree to the terms of the Academy’s computing and technology policies.
Field Trip Permission Slip - Authorize participation in Academy field trips throughout the school year.
Dining Points - Purchase dining points to be used with BU Terrier Card; cards may be refilled throughout the school year.
MBTA Passes - Information on MBTA passes for students.
Hometown News – Authorize the Academy and the University to release and publish student achievements and accomplishment (new students only).
Dietary Restrictions Form- Inform the Academy of any allergies or special diets.
